A Backflow Prevention Cross-Connection Survey form was included in your February Utility Bill. Back flow Prevention and Cross-Connection is a State mandated program. The information from these surveys is used to protect the public water system from possible contamination. It is mandatory that these surveys be submitted by all City Water Customers every 5 years.
If you have not returned your survey, please complete and return the survey. If you have misplaced the survey, you can print the survey, complete it and return it. If we do not receive a sufficient number of completed surveys, the Utility may be required to go door to door to inspect properties and complete the survey information.
